He can see with his own eyes the effect that they have on Malcolm. It's not really the kind of 'functional' that Doc was expecting.
While he doesn't verbally acknowledge Negan's hand in the matter, it doesn't mean he didn't hear it, or appreciate that he had been there when it mattered. Doc just needs a while before they're back on some kind of tolerable footing.
